Accomplished healthcare executive will join the board of two Redesign Health Operating Companies
We’re thrilled to welcome visionary healthcare exec Joseph Jasser, MD, MBA as our newest Venture Chair. Joe brings 20-plus years of leadership experience with some of the industry’s most successful incumbents and startups, including Humana, Signify Health, and Cleerly. At Humana, he helped spearhead the creation of Conviva Care Centers, a value-based approach to senior primary care that now includes 135 centers across Florida and Texas.
Creating better results for patients and ACOs
In his role as VC, Joe will also serve on the boards of two Operating Companies built at Redesign Health: Cascala Health and a company still in stealth mode.
Cascala Health is developing a comprehensive care transition platform that will help providers and patients bridge the communication gaps between hospital discharge and post-acute care. Cascala is built to solve the pain points that accountable care organizations (ACOs) have traditionally faced around transitional care management (TCM). Through data sharing and analytics, Cascala will empower patients to manage their care, creating healthier populations and reducing healthcare costs.
“Improving data access can help providers cut through the noise and find the one or two strategies that will work best in the ambulatory space,” Joe says. “Then, we can incentivize those strategies so physicians can optimize their TCM strategies and get rewarded for keeping patients healthier and out of the hospital.”
In addition to working with Cascala, Jasser will advise the leadership team of a stealth operating company developing a tech-enabled, human-powered solution to help patients manage serious illnesses. The solution will identify patients in need, help providers engage patients and their caregivers, assess patient needs and goals of care, manage patients and their needs, and ensure timely and appropriate referrals.
